Insurance Information:

We are a Participating office, for most covered services, with Blue Cross Blue Shield, Blue Care Network, Health Plus of Michigan, PPOM, Hcap, Aetna, PHCS, McLaren, Cigna, TRICARE and Medicare. Present your insurance card to us at the time of your appointment so we can bill the insurance, if it is one we participate with. If you are unsure if we participate with your insurance, please ask us. We will bill for your covered services. Co-pays and deductibles are the responsibility of the patient and expected on date of service.

If you have Health Plus of Michigan, Blue Care Network, Aetna MC or TRICARE Prime, YOU MUST OBTAIN A REFERRAL from your Primary Care Physician to be seen. We will bill for your approved services. Co-Pay and deductibles are the responsibility of the patient. Ongoing care requires authorization from your Primary Care Physician (PCP).

If you have insurance that requires you to have a referral, we must have it in our office at least ONE WEEK PRIOR to your appointment.

Credit Cards - Mastercard and Visa are accepted for office visits and payments on monthly statements via the mail.
